Is there an extension to allow the user to input text on product page?

One client wants to allow the user to input some text on the product page if required (If they want extra salad ingredients or want any removed). Is there an extension to do this?

Obviously the "textarea" and "editor" custom fields which I've tried don't work as they contain text defined by the website, not the user and cannot be changed.

Thanks this is similar to what is required, but is there one for a textarea rather than just an input field?
