Having trouble with usps shipping module. Only appearing one shipping rate

Using joomla 1.5 and virtuemart 1.1.6 stable

We have gone through the steps of setting up USPS shipping module

from installing the updated modules to entering in the log in and passwords and making sure there are weights on our products.

Now when you buy something it go to the check out 2 and the shipping choice comes up with no check box to choose the shipping modules usps.

Not sure what is going on it use to show up but now you are stuck on check out 2 because you are not able to chose a option.

Even using other options or standard shipping no box comes up.

In a nutshell I need the Priority Flat Box Rates (small, medium and large) and nothing is appearing when trying to checkout at the shipping part.  Do anyone have a usps version that will work for Joomla 1.5 & VM 1.1.6?  Need help ASAP and if anyone have a positive direction on this matter your help will be greatly appreciated.
