Intermittent add to cart failure

Adding products to my cart has become an intermittent problem.
 
1.  Click "add to cart".  The white box pops up. 
2.  Click "show cart".  The page moves on and shows the cart.
 
Here's the problem:-

Sometimes the cart comes up with the product added.  Sometimes the cart comes up without the product added!!!!!!!   
 
It is not product related.  A product that fails one time will be OK the next.
 
AAAAARRRRRGGGGGHHHH

I have tried:
1. Disable Jquery in vmart template admin - which seemed to make it fail 100% of attempts
2. Disabled "Display Modal POP UP on add to cart". - which made the site go straight from "add to cart" to the cart with a success message if product added OK - but the process still fails frequently.

I think it may be related to slow database response times. 

Any suggestions would be really appreciated.

You must use html entities for product names. for example instead of & you must use &

The reason is that people want to be able to style their product name with html.
