Duplicate Fields in checkout

How do I remove the duplicate title, country, and state fields?

See screenshots showing that there are not duplicate Shopper Fields.

You have certainly 2 javascript to render "styled" lists, it's why you have 2 lists
Disable 1 script adding list .jq-selectbox should solve the problem See https://github.com/Dimox/jQueryFormStyler/
