Need new dropdowns with DB lookup in checkout

I have a client who needs the following added to checkout

They run a business where a portion of their proceeds are donated to a charity.  They need the customer to be able to pick the charity on checkout.

When a user enters their address, we need an additional series of dropdown boxes.  They will pick a state, city, and then charity business (single field of name and address) which will be saved with that order. 

This selection should be visible on the VM order page, the customers account order information, and the order email.

They do not need the ability to edit it later.

Wanted to add that the system should use the existing state table, we will need a table added for the city and shelter locations within that city.  We'll populate the database obviously...

Because I've gotten some strange questions...  Here is the process:

1. User clicks checkout
2. User enters their shipping address

AT THE BOTTOM AND SEPARATE FROM SHIPPING:

There is a separate box Labeled "Select Charity"
3. User selects a state from a dropdown
4. System reads cities in that state from a db table we populate
5. User selects city.
6. System reads available charity locations in that city (from a db table we populate)
7. User selects charity location and it saves with their order.
