Mixed up payment and shipment methods on order placement

Hi,

I'm running VM 4.2.0 on J 3.10.12 and I'm facing a very very strange issue...

Scenario1: New user using guest checkout for the first time

All payment and shipping methods are correctly displayed on checkout and are selectable just fine.
After placing the order, the "Thank you" message also displays the selected payment method.
The order stored has correct payment and shipping info

Scenario2: The same user puts a new order

Their billing and shipping details are already shown in the checkout.
All payment and shipping methods are correctly displayed on checkout and are selectable just fine.
After placing the order, the "Thank you" message also displays the selected payment method.
The order stored has incorrect payment and shipping info.
The order stored has incorrect articles in order.
The order stored has incorrect product weight.

I thought that disabling "Save shopping carts as cookie" would remedy this but with no luck. Same thing with clearing the carts table in the db...
Tried using the default VM checkout as well as VP One page checkout but same problem.

Any idea on what might be causing this?
