Issue with title tags

Hi

My custom page titles are not working correctly. Looking at the source code, I see there is a meta title tag using my custom title, then a default store title tag. The second one is overriding the first one, which is the one that should be used.

See

   <meta name="title" content="Custom page title" />
   <meta name="description" content="XYZ" />
   <meta name="generator" content="Joomla! - Open Source Content Management" />
   <title>Default page title</title>

Would really appreciate some advice on this.
