A form instead of classic Attributes

Hey guys,

do you know about any plugin or extension for VM, which can give me a possibility to make a form with product attributes? My problem is, that I need create a 14 custom fields with about 250 products attributes for almost 2000 products. A good 60% of all atributes don't change a price like "Male/Female" or additional queston "Dog race", so in VM3 it looks really awful when it shows "Male Free/ Female Free" -> this problem I solved by removing of translation "Free". But still I need to make some form, which can a customer send with every product, form, which can I place anywhere in product description (Is not forced to be just on the right next to the photo).

In VM1 it was easy and quiet nice goo.gl/AFp9wC but in version 3 attributes look like a super NOODLE with product description in the end goo.gl/cjyttK
