[SOLVED]Offline Credit Card Processing Missing Radio button and Card drop down

Payment module Offline Credit Card Processing alatak_creditcard v1.10

Clean install with sample data

As per attached screenshot:
There is no radio button to select the CreditCard Payment
Also no drop down to select Credit Card Type

(I have just upgraded to this version of the payment module
The reason for upgrading was that with my previous version v1.4 the radio and drop down were available but when shopper selected/saved the payment the cart view still said 'No payment selected' even though 'Confirm purchase' was available and the order could be placed.  I was hoping to cure this problem but instead found the new problem!)

All sorted by with a new version which was quiclky released as soon as I submitted my ticket.
