VM Custom filed calculator

Hi guys,

I have i little problem. I have in VM price calculator but i wanna create my own new with different fields. I really dont know how can I create calculator like the same I have,bcs i paid for my website to company.

I'll send you attachment with my calculator, i want to have the same but with other fields thx to all and sorry for my bad english.
