delivery address is not addable anymore

Hi,

I do have some "big" issues about the delivery address in VM2..
Only on one page is the delivery address addable or even insertable.
The VM Version where this is working is the Version  2.0.20b.

On all other pages where i use VM is this Feature not working......
One has 2.0.22, 2.0.14 and so on. But i'n not sure if this is the reason.

When i try to add a delivery address with a Gest Check out.. I see only

"Address adding/editing"
save - cancel

!! But no fields to but any infos in.

And when i logg in as a user I get this:

        vmError: Table userinfos check failed: address_type ST without name
        vmError:
        vmError: Konnte die VirtueMart Benutzerdaten nicht speichern
        vmError: VmTable #__virtuemart_vendors_de_de Check not passed. Neither slug nor obligatory value at vendor_store_name for auto slug creation is given

Any clou?
