Tax Calculation on Orders

I need some help configuring taxes on products sold, but I am having some trouble.  I only want a 6% tax applied to all items bought by somebody residing within the state of Maryland, United States.  For all other states the tax should be Zero.  I created a Tax rule with Country: United states and State/Province/Region for Maryland; but the tax is being applied to ALL orders.

This tax amount shows up on the Product page and the Shopping Cart, even before a shipping address is selected.

1. Any ideas on how to solve this, so the tax is only applied After the Shipping Address is entered and Maryland is chosen?

2. Also don't want the Tax to be shown on the Product page.  Just the base price.  How can this be done?
