New Custom field type charge to cart once

I would like to create a new custom field type to charge once the total price of cart and not the price of the product. (default custom field charge as (product_price + custom_field) * quantity).

I need the new custom field type to charge as  (product_price * quantity) + customfield.

any suggestion? or any commercial plugin?
