Taxes charging incorrectly

I have a Texas sales tax setup per bill for 8.25%. This works correctly most of the time. However, it also sometimes does not charge Texas residents and does charge non-Texas residents. I currently have an order from Malaysia that was charged Texas sales tax. I have been through everything and cannot figure this one out. Anyone have any ideas?
