Errors: 1) duplicate announcements from VM2 2) when edit user from cart

Got a couple errors that could be from the same problem. 

1) when add an item to the cart on the user end, or when I do something in the backend, Virtuemart tells me the results 2 times.  (Like Picture 2.png that's attached).

2) Every time I make a change to my user status from the front-end, in the cart when I'm making a purchase, I get some kind of error.  Picture 1.png is the most recent error.

Any suggestions?  If it's a table error, which table entry would I edit?
