Custom Fields on Checkout

hi, i need two more fields on checkout:

1. (Boolean)  send as present Yes or No
2. (Text) write a personal text for your gift card
3. (date of delivery) choose the date of delivery

i tried to crate 3 custom fields. but i can only connect this fields with the product and not to the check out form.
i also tried to find some informations on the internet how to add customs fields to checkout form.

but no results...

Look in the "left menu"

"Configuration"
"Shopper Fields"

Those are you checkout billing/shipping fields

I am surprised you had to report this thread 9 months later.
