Cannot Add Product Categories, Manufacturers, Manufacturer Categories, etc.

This is what the screen appears whenever I try to ADD a Product Category, Manufacturer, Manufacturer Category and more...I can add products all day, but cannot add product categories...PLEASE HELP!!!

I'm running Joomla 2.5.4, Virtuemart 2.0.0 (Cannot get 2.0.6 to install or update), PHP version 5.2.14 running on Apache.
