Add simple form to invoice

Hi there,

I'm currently busy with a webshop in Joomla 1.7.5 and the latest Virtuemart extension and I was wondering if it was possible for the following.

The shop is going te be a site where people can search or look up a country and businesses within this country.

For example:

Company: Techcom
Country: Germany

Since the search is done on another website (portal) the people who are interested can buy a report about this company via VM.

Al they have to do is fill in a form:

- Company Name
- Country
- Location
- Registry No.

If they fill this in my customer knows to look for.

Is it possible to add the form with the 4 searches to the invoice?

So that the customer can see which company searches were done, with which speed and the type of report.

Any ideas?
