How to configure Quantity related price calculation extension?

I purchased the Quantity Related Price Calculation extension and am just a little confused on how to configure it for what I need.

Such as:

Less than $499.99 -> 0% Discount -> then add 20% to total for freight (after the discount)
$500-$999.99 -> 5% Discount -> then add 15% to total for freight (after the discount)
$1,000-$2,499.99 -> 10% Discount -> add 12% to total for freight (after the discount)
$2,500-$4,999.99 -> 15% Discount -> add 10% to total for freight (after the discount)
$5,000 and up -> 20% Discount -> then add 5% to total for freight (after the discount)

Also, orders less than $150 are subject to a $15 shipping surcharge PLUS 20% of the total for freight.

Is there a really intelligent person out there that can guide me in the right direction?
