Unique shipping charge for rural delivery

Hi all,
I need to apply an extra shipping fee for clients that live in a "Rural Delivery" area, I can setup a new shipping method but I don't know how I would apply that to this group of clients that live in a "Rural Delivery" area. Does it require another field in the shipping address for where the client for tick a box for example that would say "Rural Delivery"? This "Rural Delivery" would be on top of the shipping cost by weight for each item.
