Add optional insurance cost to cart

I would like to add an optional "insurance" voice to the cart (basically is an additional cost for some shipping methods, that the user may or may not enable).
I guess this may be handled by a plugin, but before starting to code I would like to know if anyone did already something about it.

I am curious to know if someone actually implemented this.

We simply want to offer an additional insurance without it being tied into a particular carrier. For example, if the person selects "Yes", then the'll have to sign for package or if the person says "No" then they are responsible for lost or stolen package.

This is a very, very basic plugin that automatically adds a product to the cart. E.g. you create a "Insurance" product and it will always be added to the cart as soon as the user starts the checkout.
Of course the user is able to remove it from cart and it will not be added again in the same session.
