Custom Field for shopping cart page

Hi Admin,

I just want to ask how can I add custom field in the shopping cart page above the check out button and it also add value to the the total price.

For example:
radio/check button - Free Shipping (Free)
radio/check button - 1day Shipping ( Fee: $3.00 will be added to the total price)

Is there any plugin that can add custom fields before checkout button or I need to customized the form itself from the plugin?

Please help me. I want to add option(radio button/ dropdown) on my cart page that will add additional value to the total price.
for example:

1. Free pick-up (No additional charge)
2. Two days delivery( Additional charge).

no versions etc .. sigh..

what do you mean cart page?  Checkout cart?

Custom fields have nothing to do with this - just use the native weight/zipcode shipping tp have a free method and a pay method.
