Searching for extension, which allows to attach files during checkout per ORDER

i need an extension, which allows to attach files during checkout.
Everything that i found, allows only the shop owners to attach multiple files on the product details page.

I need the buyers (users) can attach files during checkout per order. For example, this way: http://joxi.net/Drlan40IjZJJmP
