How to add instructions/notes per item at checkout?

Is it possible to give the customer the ability to add special notes/instructions per item at checkout? Say, for example, they want to have Book A signed with a short message, but they want Book B unsigned with no message. I'm thinking having a notes/instruction textbox per item at checkout would be perfect!
