Customized shipping rates with USPS

I'm not sure how to make the shipping rates for USPS customized. Is this possible? I have attached a photo with the email that my client wrote to me with their personal shipping rates.

Sounds like you need a custom shipping module made.  I can do this if needed, for a fee.
