Customsize plugin configuration

I have purchased and installed the Customsize plugin, but am having difficulty in getting it to work, and sometimes to display.
I'm looking to put in dropdown lists so can't see how to obtain this, also the grid to enter the options, how does this work because it seems to not have any effect, I have followed the instruction video steps, but it really just shows you very basic, cannot seem to find any other details on how this plug in fully works, does anyone have experience with this they could give me some advise please.

I'm looking to have two dropdowns, one with size, the other with quantity, then obviously the final price is dependent on the size/qty selected. Every single combination has a different price.
