VM 3.0.12 admin links missing "administrator"

VM Ver: 3.0.12
Joomla Ver: 3.4.8
Apache Ver: 2.4.18
PHP Ver: 5.5.30

I have tried a clean install of 3.0.12 as well as removing all database entries and installing an older version of VM (which worked fine) then installing 3.0.12 over the older version, yet the admin links are not generated correctly in the back end. For example, the 'Product Categories' link should look like this:

http://www.domainname.com/administrator/index.php?option=com_virtuemart&view=category

but if looks like this:

http://www.domainname.com/index.php?option=com_virtuemart&view=category


I can load the proper pages in the back end by adding "administrator/" where it is missing, however this doesn't fix all the issues. For instance, under Shop > Shopper Information [tab], None of the States appear after I select "United States" as the country. I have searched for posts where others have had these issues, but I have not found any. Can anyone offer some insight here?

At this point, I believe it's a javascript error but I have not been able to resolve it.

Not sure how I fixed the issue, but after months of uninstalling, removing database tables, and reinstalling, this is finally working as is should.
