When installing from upload or tmp folder I keep getting a 500 error. When checking the server error log I see:

2015-03-05T20:42:51+00:00 INFO 127.0.0.1 joomlafailure Username and password do not match or you do not have an account yet.

I have no issues with any other installs or updates. VM appears broken on my site.

Please advise.

Is a Joomla error, maybe something preventing installation like anti-virus can do on localhost. Google the error and you will see a lot of such reports with all sorts of joomla extensions.
