Title: Ask a question popup not working

Hi, I'm using virtue mart version 2.6.14 along with joomla version 2.5.28

When I go to product pages on my site and open any of the product. Here is ask a question button which on clicking pops up a 404 page,instead of a form.
Please help me to solve this issue.
