Print Page - A Purchase Order payment method

Joomla 2.5 and VM 2.0

Ideally I would like a payment method called Purchase order with a field next to it for the user to enter the PO#.

Is there a way to do this? Do I need a plugin?

My only way around this temporarily it appears is to create a payment method named Purchase Order and direct the user to enter the PO# on the "Notes" comment box...
How do I change the title, "Notes and Comments:" above the input box?
