Print Page - Empty "ship to"

Hi,

I'm using VM 2.0.18c.

I have problem with Ship To address. I can fill bill to form - everything is ok, but when I click "Add ship to" button ( link provides to: xxx/index.php/component/virtuemart/user/editaddresscartST?cid[0]=0&Itemid=0 ) there aren't fields.

Please help me :)
