I just placed a test order on my website, when I clicked on 'confirm order' it showed the cart page and said that my cart was empty.
The order did not go through.
Normally the orders do go through correctly. I have also seen that sometimes when I click on ADD TO CART - it takes me to the cart page, but says that the cart is empty.
I am using One Page Checkout and a custom template with VM 2.0.8e
Not sure what to do about this as I can't always replicate the error.

Looks to me like your additional extensions are loading too many (conflicting) instances of jquery. The tool described here will probably help you to sort out the problems:
