I have been looking for this module since January. It appears to work fine. I ordered it and they even give you info for sandbox.paypal.com if you don't have your own sandbox account. You set it up almost the same as you do the standard paypal module that comes with VM2.
I needed this so customers can use their credit card directly on the web site without having to go to paypal's site.

since the last post the module has been updated several times, it seems to work internationality for most countries, and we released a separate version that works specifically with paypalpro italy
we also released our new mail order manager for virtuemart, a VM 2.0 upgrade from our popular add orders extension for VM 1.1.x series
2.0 installs as a component and works with the paypalpro module (an other payment methods)
now store manager can add orders while their customers are on the phone, and even use it as a POS system
