alatak_usps Shipping Plugin not displaying

We have problems with alatak_usps. (I am unable to get help at extension.virtuemart.net due to lost password, see bottom of this post)

The problem we are having is here:

Once we connected our USPS user name the shipping won't work anymore. In fact it stops our checkout from working at all (we do have a free pickup option).

If I disable alatak_usps our "free pickup option" will show. But with it enabled I never get to the cart page.

With enabled upon placing something in the cart I go here:

With disabled I go here:

We have "One page checkout" enabled.
We have "Only registered users can checkout" disabled.

I have cleared browser and Joomla/Virtue mart cache during all my tests.
We are set in Production mode.
Verify Address is set to no.
I have read through all the docs and every single forum post I could find.
Have reinstalled the plugin.
I have tried switching Joomla templates.
I have shipping method sort orders set.
It is set to ship by weight and product weight is set for sample product.

and created menu item for account and allowed user registration. No Help.

I set shipment method to "VM Shipment - by weight, Zip and countries" and they shipping shows but doesn't work. I then switched back to alatak_usps and then it worked, but not the next morning. It is back to not working.

We just purchased alatak_usps on January 3rd, 2018 but lost our password.
We are trying to put in a ticket at https://extensions.virtuemart.net but when we choose "lost password" we don't get the email to reset the password. Please let me know how to reset the password. I emailed the developer but got no response.
I suspect earthlink is blocking your incoming email so we might need another solution.

that didn't work, the account that lost the password is not associated with the account I am using here. I didn't include the email here as it is private. I responded to the email to include the correct email address/account that needs the password reset.
