Module suddenly stopped working

My website sells 1 item which is 60 lbs.  I am using the standard shipping module that's set up for free shipping, but if they want the item overnight they can pay to order it thru UPS. This has worked fine for the last few months, but all of a sudden the UPS module no longer pulls rates.  It doesn't even give me an error, it just says overnight shipping is ($0.00).  This is a HUGE issue!  I haven't been able to locate the cause while looking through the forum. Can anyone point me in the right direction?  VM 1.1.3 on Joomla 1.8.

Put your shop in debug mode.  Run through a test checkout.  Let us know what it shows.
