custom field - textbox

I am creating a website that sells business cards so as a part of the custom field I need a place for the customer to type in the name they want on the card, their phone number, etc. How do I get that in a custom field. I am assuming that it needs to show up in the cart in order for me to see it to create the business card for them. I have seen some websites that have a button that says "personalize" and then a pop up box comes up with text boxes to put all the info in and then it saves to the cart. I don't know if that would be easier. Any help would be great. I just need pointing in the right direction.
