Title: Bug: Manage User Fields (deleting a field)

When deleting a User Field, the rows in the jos_vm_userfield table are correctly deleted but any entries in the jos_vm_userfield_values table are not and become orphaned. (vm 1.1.2 nightly 3rd Aug)
