We have a few specific products, to which we want to add an additional shipping cost (due to size/weight).
The additional cost has to appear as a separate shipping cost (* customers picking these products up from the shop will not have to pay this additional cost).
What is the proper way to do this?

Not exactly what you are asking for, but I would try it with creating three shipping methods:
1. Pick up in our shop (free)
2. Standard shipping cost
3. Assign a certain weight or size to heavy/large products and assign a minimum weight or size to another shipping method, so that the higher shipping cost is automatically selected when one of these heavy/large products is added to the cart.

Thanks. I see the following potential issues with this approach:
1) If the customer orders many items for the "heavy" product, we want the extra shipping cost to be per item (and not added just once)
2) We don't want to introduce multiple similar shipping methods in the cart, and confuse the customers. Delivery to customer address, should be a single option to select. Only the cost should change.
