Customer attach a file upon checkout

Hello,

Is there an extension that allows the customer to attach a file upon checkout? I would like the attached file to be sent to the administrator's email address.

If there are no extensions out there, could you please re-direct me anywhere where I can get help to manually install the code to implement the above?
