Fedex Only Home Delivery no other choices

J 1.5.9  vm 1.1.3  sh404SEF otherwise no other mods

Finally got fedex working except the only choice that it gives is Home Delivery.
If I dont select home delivery then I dont get any choices as picture shows.

I have given it known business addresses and the same happens.

This is the info you get when registering with fedex, plus acct number.
Supported Web Services: FedEx Web Services for Shipping
Authentication Key: RyEXicwiRAT27wHB
Meter number: 190622183   

So I used My fedex acct number and the meter number in the setup.

Authentication key un-needed?

Does the signature option matter?
Is a residential address determined by a blank company name?

I do have a custom template for the cart could it be that?
