I hacked the cart modul, but I dont know where to start to update/delete the attributes in the cart.
The cart is saved in a session. Maybe its easy to manipulate the session for delete the attributes?

Yes, the cart contents is in an array stored in SESSION variable. Better you manipulate that directly.

You can save the cart's content in an array:

$cart = $_SESSION['cart'];then you can examine, change , basically do whatewer you want with.

I fixed the error, was the problem in virtuemart css, but I wonder if there is the option to hide the checkboxs and when I click on the second taste of the pizza, they would appear on another page. Example I click to choose a second flavor, and the page refreshes, and then he gives me the option to choose additional (checkbox).

Short answer: no.
The entire hack relies/piggybacks on VM's cart code. You can't add page reloads, etc here withouth hacking deeper in the VM code.
But you can add some CSS /JavaScript based trickery to show/hidden divs based on user actions.

I managed to solve the problem, I created child products. I just wanted to know if I can use UTF-8 in multiselect or select your hack because I'm having problems with Portuguese words. ex: Requeijão the problem is "ã".

Is not the hack itself, it's the VM cart code which causes problems. Check the code, in one of attributes (Registration, if I remember well) I allready implemented a trick to workaround the problem. Is described in one of previous posts.
