Invalid Country Selection Error. Only ship to ONE country!

VM 2.0.25

This has happened intermittently:
Users would attempt to check out and there would be a dialog box popping up saying that customers are unable to ship to that country.

#1: There is no Country selector
#2: Default Country is United States
#3: Country Selector field was set to 'Unpublished' and 'Not Required'

So then how can that error pop up when the field was unpublished to begin with?

A little confused and could use some help troubleshooting.

The chances of getting help with a 2.5 year old, out of date test version of VM, I would have thought was nil.

the latest (last?) version of VM2 is 2.6.22

Is there a country set up in the shipping plugin? That's about the only thing that I can think of for that old version.
