Tax rules not applied after upgrade - this only happens sometimes!

Hi there,

We've encountered a pretty ugly issue after upgrading to VM 3.0.6.2 ( from VM2 ), i.e sometimes the tax rule that qualifies to be applied to an order is not being applied and the customer ends up paying the product without the relevant TAX applied.

This happens rarely, like once in 2 months maybe and we wasn't able to replicate it in our test orders. We've even tried to replicate by creating orders with identical information ( user info, product info, etc ) to the orders that did not pay the relevant TAX - and all went fine with our tests, the TAX was applied to the order.

We also have the OPC (https://www.rupostel.com/) component installed - not sure if the issue is related to this component though.

Any idea how this bug can be detected/replicated? Any advice would be appreciated!
