Multiple Quantity Options

Does anybody know how to do something like this?

I'm setting up a clothing shop and it needs the ability to select a quantity for each size, ie: Small, Medium, Large, Extra Large, etc.

In other words they will be ordering multiples of the same shirt, but different sizes.
