Add on Items - Off Main

How can I add an add on item, scenario below:

Main Item: Spaghetti & Meatballs $6:99

I want the user to be able to add Garlic Bread: $2.00

Thank you for your response.

I tried but could not get it to show prices or even show on the item.

Also, this will be different for every item, some many not have bread but may have baked potato and then some items I will want on all like Beverage?

You can apply different custom fields to different product. Have a look at http://forum.virtuemart.net/index.php?topic=117884.0 for a plugin that makes custom fields easier to use & display.
