How to add price to Custom Attribute?

Hi. We're using VM 1.1.9 for a t-shirt store, and some of the t-shirts you're able to add your name to the back of the shirt. For the shirt we have a "Size" in the Attribute List, and then down in the Custom Attribute List we added "Name", so for the shirt we have the Size dropdown and the Name text field.

If the customer wants to add a name to the back of the shirt it's an extra $5. How can we show and have the price reflect the $5 if the customer types their name in that field?

If it's not possible to add a price to an attribute in the Custom Attribute List is there a workaround?  Maybe add a "Name on shirt: No / Yes ($5)" list attribute and then just have the "Name" custom attribute below that?
