Product Image Upload Box Mucked Up

I'm running VM 1.9 and Joom 1.5  http://rocksfast.com

Site been running well for over a year.  Now whenever i go to add or change a product image the box that pops up shows the current images for that product.  I click NEW to add a new product image and the following page with the browse button to locate image on Hard Drive for upload....it's all messed up.  There used to be options for thumbnail size, main image size, image name, a PUBLISH checkbox etc....now none of that is there.  It's all just blank and messed up.  I tried adding images to products in different categories...no go.  Tried firefox and IE.  Updated JAVA.  Same thing.  Any ideas?  I did upgrade VM to 1.9 about a month ago if that helps at all.  plz advise.
