When Is always ?
This week, last day, last Year, or something else ?

You are using old and insecure Joomla and VM versions ....

I can open the form, see attachment.

What changed last week ?

You say ALWAYS, why can I open the form ?

Errors come from changes.

i change mailconfirmed.php, add google analytics event code

but i restored backup of complete folders of website before change and problem persist.

other thing i changed is shopper info (virtuemart -> shop -> shopper info) name, last name and address but thinking can be i restored this data.

change or add code mailconfirmed.php is in attached image

Check You changes once again.

You are in this case absolutely best suited to know what You changed and when the error came.

My advice would be what You have done, but there is probably more to it than this. Recheck Your changes, it is hard to get support on a VM version that is that old and insecure.
