Option to add personalized text - but make text mandatory if you choose (yes)

OK, I have an item that can be personalized with up to 2 lines (60 characters) of text for an additional $2. How would I?

1) Have it require the text if (and only if) I choose yes from the personalization attribute?

2) Limit the value of the personalization attribute to no more than 60 characters?
