Should this not be easy to set up in Virtuemart? Anytime i see anyone wanting to do something similar they are told they have to make a custom shipping module, which i would assume is somewhat complicated.

Does anyone know an easy way to do this, or a tutorial that can help me in the right direction?

I would also be happy if i could just get 5 dollar shipping per item, flat rate.

You can do the per item flat rate with the Zone shipping module. You can also set a max value. If all countries are the same then you only need the default

No, you don't need to make any edits.

Take a look at the Store, Shipping Modules and the configuration options for the Zone shipping method. If you would be happy with a per item rate of $5, then set your zone cost per item to that. You can set an upper maximum if you want to (zone cost limit), so that no-one will have to pay more than that amount in shipping charges.
If you put this against the Default Zone, it will apply to all countries, unless you assign them to another zone (in which case, you would need to set up your per item shipping rate for that zone too).
